JEVS Care at Home is excited to announce a new home care option for Medicare participants with Dementia. We have partnered with PocketRN to offer the GUIDE program, a Medicare benefit that brings dementia support into the home at no cost to eligible individuals.

What Is GUIDE?

GUIDE stands for Guiding an Improved Dementia Experience. It is a program created by Medicare to help people living with dementia and the family members who care for them get more support at home. GUIDE includes nurse-led care planning, caregiver education, and ongoing guidance. If you qualify, GUIDE is covered by traditional Medicare Parts A and B.

Why This Matters

Many families assume that Medicare does not pay for home care. So, they either pay privately for help or try to manage everything alone. GUIDE was built to change that. It gives families an option that does not depend on their ability to pay out of pocket.

What GUIDE Includes

For eligible individuals enrolled in traditional Medicare, GUIDE offers:

  • Personalized care planning and ongoing monitoring
  • Medication and care navigation support
  • Access to a care team, day or night
  • Caregiver education, coaching, and resources
  • Up to 76 hours of in-home respite care for qualifying families, based on GUIDE’s annual respite services amount

Who Can Qualify

You or your loved one may be eligible for GUIDE if they:

  • Have a dementia diagnosis (we can help with this if needed)
  • Are enrolled in Traditional Medicare Parts A and B
  • Are not currently enrolled in hospice
  • Live at home, including independent living communities

People who are dually eligible for Medicaid, enrolled in palliative care, or covered by VA Benefits may also qualify. There is no obligation to enroll in GUIDE if you apply. Every family’s eligibility is reviewed individually before enrollment.
